Memorial Scholarship Application

Criteria for Scholarship Applicant

1. Applicant must be an active member, in good standing, of a
recognized emergency services organization in the State of Indiana
for a minimum of one (1) year.

 

2. Applicant must apply the scholarship award toward an accredited
college/university, Emergency service related program in the state
of Indiana for the purposes of furthering their skills and education.

 

3. Once a scholarship is awarded to an applicant, they will not be
eligible for future scholarships through this program.

 

4. The applicant must maintain the scholastic standards of the program
in which they are enrolled.

 

5. The award will be paid to the school of the student’s choice and the
funds must be used within one year of the award date. Any unused
portion of the award will be returned to the Memorial Scholarship
Fund.

 

6. Scholarships each will be awarded annually provided that qualified applicants and funds are
available.

Administrative Requirements

1. Applications are available through any committee member of
the Duneland School of Emergency Response or on our
website.

 

2. The applicant must provide complete and accurate
information as requested. The application must be filled out
in its entirety.

 

3. The address of the Duneland School of Emergency Response
is P.O. Box 1033, Chesterton, IN 46304-0133.

 

4. The entire application is to be mailed to the above address,
postmarked no later than March 14th of 2009
